A Bridge between EHRs and Practice Management Systems

The broad application of this technology applies to not only patient care, but can also help sustain the financial health of many rheumatology practices. New legislation proposes to dramatically shift how physicians are reimbursed. The JointMan® platform can build the bridge between Electronic Health Records (EHR) and Practice Management systems that may lack the versatility needed to comply with the upcoming shift to outcome/value-based reimbursement. This is a defining moment for rheumatologists—together, we can bring about significant change in patient care through successful collaboration at the local, regional, and national level.

Clinical Decision Support & Care Coordination

Clinical Decision Support & Care Coordination

JointMan® Clinical Support supports best practices in Rheumatologic patient care. This scalable, comprehensive decision support platform allows rheumatologists to track patients’ progress as they strive for low disease activity. JointMan supports broad decision-making over time, tracks and monitors biologic effectiveness, and records all discontinuation reasons, including adverse events.

  • Uses a color-coded anatomical (homunculus) diagram tied to calculated algorithms which then generates a weighted, trended analysis that includes clinically observed joint pain, swelling, labs and physician VAS.
  • Gives real-time access to patient/physician-reported metrics (DAS, CDAI, Rapid 3, etc.).
  • Identifies clinical variation, gathers clinical evidence, and eases the conduction of assessments.
  • Provides a broad view of the patient’s health, including risk stratification associated with documented co-morbidities.
  • Lends itself to making informed clinical decisions regarding therapeutic response, discerning which patients are at risk of joint damage, and identifying patients who are in clinical remission.
  • Bolsters care coordination by streamlining records, treatment plans, and other valuable data support back to the primary care physician.

Reimbursement and Patient Access Support

Reimbursement and Patient Access Support

JointMan® Reimbursement and Patient Access Support provides solutions that significantly bolster practice efficiency. Transforming data into insights and intelligence, delivered where and when they are needed, will ultimately improve patient outcomes and demonstrate cost savings. Early and appropriate treatment can prevent costly treatment in the long term by managing the disease process actively and appropriately.

The advantage of using JointMan is that for the first time in history, Payers have all of the treatment information and outcomes needed to analyze and best manage their benefit package. Not only will they have the ability to understand and manage their members’ risk stratification early on, but they will also have access to documented proof that treatment is efficacious for each patient. In short, JointMan provides an assurance that the right care is being prescribed.

  • Increases the financial health of the practice by providing data that facilitates value/outcome-based reimbursement negotiations with public and private payers.
  • Decreases the hassle factors associated with the administrative burden of pre-authorization and denial letters.
  • Imparts clinical and financial transparency to Payers so that they can better understand the quality of treatment, and thereby more efficiently manage claims.
  • Provides Payers with pricing on both an individual cycle basis, and on an aggregate basis, for better comprehension of the total claim cost.
  • Gives additional Medicare PQRS reimbursement support.

Clinical Trials & Screening Tools

Clinical Trials & Screening Tools

JointMan® Clinical Trials and Screening Tools provides a one-stop shop for real-world investigator and clinical studies. Incorporating JointMan within the clinical practice accelerates patient recruitment, reduces study start-up timelines, helps screen potential patients, and is a universal data collection tool for both clinic and study-related assessments. JointMan aggregates the inclusion and exclusion criteria of the practice’s trials so the physician is alerted of a potential patient at the point of care.

  • Expedites more efficient protocol feasibility assessment
  • Optimizes drug safety monitoring
  • Consolidates EHR and Research data input within the clinic
  • Facilitates faster patient access to innovative medicines

Reputation Management

Reputation Management

JointMan® Reputation Management recognizes participating physicians’ achievement, and rewards clinicians who deliver superior patient care.

Our programs measure the quality of care delivered in provider practices. We place a special emphasis on managing patients with rheumatoid arthritis who are most at risk of incurring potentially avoidable complications. Our risk stratification covers all patients who are being treated with complex biologics, and tracks the efficacy of treatment to specific outcome measures so as to better promote comprehensive care delivery and a strong relationship between patients and their care teams. Physicians, nurse practitioners, and physician assistants who meet our performance benchmarks can earn a recognized logo as a participating physician practice.

Population Health Registry and Practice Dashboards

Population Health Registry and Practice Dashboards

JointMan® Population Health Registry and Practice Dashboards entail a strategic approach to care focused on improving outcomes for entire populations, rather than just those individuals seeking health care. Population health management requires organizations to define a population, identify gaps in care, stratify risks, engage patients, manage care, and measure outcomes. JointMan enables close monitoring of the various classifications of disease activity, population demographics, and quality outcomes for Rheumatoid Arthritis and associated Spondyloarthropathies. This data is the foundation for improving patient experience, improving the health of populations, and reducing the per capita cost of health care over the long term.

  • JointMan® users across the nation contribute vast amounts of de-identified patient data into a cloud-based data repository.
  • Once in the registry hub, the platform is adept at transforming structured data into insights and intelligence that correlates research data, compares effective outcomes, and generates rapid advancements in medical treatment.
  • Through the use of clear data visualization tools (dashboards), physicians are able to benchmark results with peers, are given timely feedback regarding individual patients and/or group performance, and can collaborate in comparative effectiveness research.
  • Aggregated data within the portal, which is pivotal to predictive analytical research, can also be applied to health management profiles and population health studies.
